Chapter Recognition Contact or the Chapter's designee must submit the National Recognition Nomination Submission Form by March 1. This form serves as your official nomination for all recognition awards the chapter is recommending for national award consideration.
Additionally, all nominations packets must be uploaded into the online system by March 1. Each application packet is uploaded individually through the application links for each award. For chapters that use the online system at the chapter level, the chapter may use the application that is already stored in the online system. If two applications are submitted the most recent submission will be used.
Each nominee identified on the National Recognition Nomination form will have their recognition nomination / application submitted to their region for judging in that category. Ruby nominees will be judged by all chapters.
The National Office will only forward for consideration nominations listed on this form AND that nomination has a completed application package online. Nominations not submitted using the online nomination form will not be considered for national / regional recognition.
Chapter Membership Chairs receive an email copy of all applications submitted.You may also review the current list of award applications / nominations that have been submitted online by visiting the Award Applications Submitted Page. (Access locked to Chapter Officers only). This online report is real time data and includes applications being submitted today at the chapter level.
